Occupational Therapist, Home Health

About This Role

As a Home Health Occupational Therapist at CenterWell Home Health, you will deliver personalized care to patients in their homes, focusing on rehabilitation and functional improvement. Your role is vital in helping patients regain strength, mobility, and independence.

Responsibilities

• Assess and screen patient's daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes.
• Confer with the patient's physician and other healthcare team members, participating in the development and revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients to improve or restore strength, coordination, range-of-motion, and function.
• Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
• Participate in care coordination and discharge planning activities.
• Recommend and/or design special adaptive equipment for clients to improve their living or working environments.
• Design and train staff and family members to carry out retraining programs, including daily living skills.
• Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance.
• Accurately and thoroughly document patients' care observations, interventions, and evaluation on the day services are rendered.

Requirements

• Degree from an accredited Occupational Therapy Program
• Current and unrestricted OT licensure
• Current CPR certification
• Good organizational and communication skills
• A valid driver’s license, auto insurance, and reliable transportation

Qualifications

• Degree from an accredited Occupational Therapy Program
• A minimum of six months of occupational therapy experience preferred; Home Health experience a plus.
