Occupational Therapist, Home Health

About This Role

This Occupational Therapist will deliver personalized home health care, focusing on rehabilitation and functional improvement for patients. The role involves assessing daily living skills, developing therapeutic programs, and empowering individuals to regain independence in their homes.

Responsibilities

• Assess and screen patient's daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes.
• Confer with the patient's physician and other healthcare team members to participate in the development and revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve strength, coordination, range-of-motion, and function.
• Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities and coordinate the plan with the interdisciplinary team.
• Recommend and/or design special adaptive equipment for clients.
• Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance.
• Accurately and thoroughly document patients' care observations, interventions, and evaluations on the day services are rendered.

Requirements

• Degree from an accredited Occupational Therapy Program
• Current and unrestricted OT licensure
• Current CPR certification
• Good organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Qualifications

• Degree from an accredited Occupational Therapy Program
• A minimum of six months of occupational therapy experience preferred; Home Health experience a plus.
