Occupational Therapist

About This Role

This Occupational Therapist position involves evaluating and treating patients with cognitive and physical functioning disorders, developing individualized care plans, and supporting their progress towards rehabilitation goals at Keller Oaks Healthcare Center.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders.
• Develop appropriate plan of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Administer rehabilitative, skilled, and medically necessary treatment interventions.
• Assess patient's progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness and make referrals if needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Effectively communicate with supervisors and other health team members regarding patient progress and treatment plans.
• Provide comprehensive discharge summary of services provided.
