Nurse Practitioner

About This Role

Provide effective and equitable value-based primary care as a Nurse Practitioner, focusing on adults on Medicare to promote health and prevent hospitalizations. Work independently and collaboratively with physicians within a care team model to build meaningful patient relationships.

Responsibilities

• Provide exceptional primary care to adult patients.
• Conduct office visits for routine and acute medical issues.
• Administer Annual Wellness Visits and Health Risk Assessments, ensuring accurate and specific documentation.
• Coordinate care with other providers, specialists, and agencies.
• Lead population health initiatives in coordination with the Care Team.
• Assist the care team with phone triage and outreach.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.

Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

Nice to Have

• Experience in primary care (internal medicine, geriatrics, or family medicine)
• Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments (HRAs)
• Passion for teamwork and cross-functional collaboration
• Desire to be part of an innovative population health model
• Bilingual proficiency in applicable areas
