About This Role

Conduct comprehensive health assessments and manage acute and chronic medical and behavioral conditions for patients, ensuring accurate documentation.

Responsibilities

• Conduct comprehensive assessments
• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD 10 conditions

Requirements

• Active and unrestricted Nurse Practitioner license in New Hampshire
• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Ability to complete physical requirements of the job including navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program

Nice to Have

• 1+ year of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
