Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members to assess overall health, identify barriers to care, and educate individuals on treatment plans.
Responsibilities
- Conduct in-home visits focusing on a person’s overall health and well-being
- Identify potential barriers to care during evaluations
- Perform medication and medical history reviews
- Conduct physical evaluations in health plan members’ homes
- Educate health plan members and assess their understanding of treatment plans
Requirements
- Board-certified Nurse Practitioner
Nice to Have
- Willingness to enroll for Medicaid license (with company support)
