About This Role

Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members to assess overall health, identify barriers to care, and educate individuals on treatment plans.

Responsibilities

• Conduct in-home visits focusing on a person’s overall health and well-being
• Identify potential barriers to care during evaluations
• Perform medication and medical history reviews
• Conduct physical evaluations in health plan members’ homes
• Educate health plan members and assess their understanding of treatment plans

Requirements

• Board-certified Nurse Practitioner

Nice to Have

• Willingness to enroll for Medicaid license (with company support)
