Nurse Practitioner, Advanced Practice Provider

About This Role

Provide effective and equitable value-based primary care to adults on Medicare, focusing on preventative medicine and chronic disease management. Collaborate with a multidisciplinary care team to deliver comprehensive care and build meaningful patient relationships, helping to keep patients healthy and out of the hospital.

Responsibilities

• Provision of exceptional primary care
• Conduct office visits for routine and acute issues
• Administer Annual Wellness Visits and Health Risk Assessments, requiring a holistic view of health and accurate documentation
• Coordinate care with other providers, specialists, testing facilities, and agencies
• Lead population health efforts in coordination with the care team (e.g., ensuring evidence-based screenings)
• Assist the care team with phone triage and outreach
• Educate patients on their health conditions, care plans, and treatments
• Participate in Oak Street Health promotional activities
• Conduct home visits as needed

Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family, Adult-Gerontology Primary Care, Adult, or Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

Nice to Have

• Experience in primary care (internal medicine, geriatrics, or family medicine)
• Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments (HRAs)
• Passion for teamwork and cross-functional collaboration
• Desire to be part of an innovative model focused on empirically-guided population health
• Bilingual proficiency in applicable areas
