Deliver expert-level, advanced wound care in patients' homes as a Mobile Wound Care Provider, collaborating with multidisciplinary teams to ensure optimal outcomes and redefine patient care.
Responsibilities
- Deliver expert-level, advanced wound care including allografting, surgical debridement, and management of chronic, non-healing wounds
- Partner with a multidisciplinary team for seamless care coordination and optimal outcomes
- Travel to patient homes, creating your own rhythm of patient care
- Maintain accurate patient records for assessments, treatments, and progress
- Contribute to continuous improvement and stay current with leading-edge wound care techniques
Requirements
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license
- Strong independence, communication, and organizational skills
- A passion for patient-centered care
