About This Role

Deliver expert-level, advanced wound care in patients' homes as a Mobile Wound Care Provider, collaborating with multidisciplinary teams to ensure optimal outcomes and redefine patient care.

Responsibilities

Deliver expert-level, advanced wound care including allografting, surgical debridement, and management of chronic, non-healing wounds
Partner with a multidisciplinary team for seamless care coordination and optimal outcomes
Travel to patient homes, creating your own rhythm of patient care
Maintain accurate patient records for assessments, treatments, and progress
Contribute to continuous improvement and stay current with leading-edge wound care techniques

Requirements

Active Nurse Practitioner (NP) or Physician Assistant (PA) license
Strong independence, communication, and organizational skills
A passion for patient-centered care
