Manager, Population Health

About This Role

This role drives the clinical and economic model of Population Health, focused on delivering high-quality, cost-effective care. The Manager will design and implement key Population Health priorities through programs and interventions targeted at improving patient outcomes.

Responsibilities

• Support the execution and ongoing refinement of Population Health programs
• Work closely with functional leaders (Product, Finance, Data Science) to assist with strategy and program design
• Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities
• Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures

Requirements

• 5+ years of applicable experience in consulting, healthcare, population health, or working with healthcare providers
• Ability to build trust and confidence of stakeholders
• Track record of effectively solving difficult problems
• Practical, results-oriented mindset
• Ability to manage multiple priorities while maintaining a positive attitude
• Ability to develop business cases, ROI analysis, and financial analysis
• Ability to succinctly summarize and synthesize large amounts of information
• Expert in MS Office, Google Apps, project management software

Qualifications

• Bachelor's degree
• 5+ years of applicable experience in consulting, healthcare, or population health, working with healthcare providers, or similar fields
