LTSS Service Coordinator - Case Manager

About This Role

The LTSS Service Coordinator is responsible for managing a caseload of individuals in specialized programs, facilitating Person Centered Planning, and coordinating their physical, behavioral, and long-term support needs. This role involves conducting assessments, developing support plans, and ensuring integrated care management within Charlotte County.

Responsibilities

• Conduct service coordination functions for a defined caseload of individuals in specialized programs.
• Facilitate the Person Centered Planning process, documenting member preferences, needs, and self-identified goals.
• Perform face-to-face program assessments and apply motivational interviewing techniques for evaluations and coordination.
• Identify members with potential clinical health care needs and coordinate their cases with the interdisciplinary team.
• Manage non-clinical needs of members with chronic illnesses and disabilities to ensure cost-effective utilization of long-term services and supports.
• Document short and long-term service and support goals in collaboration with the member's chosen care team.
• Identify members who would benefit from an alternative level of service or other waiver programs.
• Submit utilization/authorization requests to utilization management with supporting documentation.
• Report critical incidents to appropriate internal and external parties such as state and county agencies.
• Assist and participate in appeal or fair hearings, member grievances, appeals, and state audits.

Requirements

• BA/BS degree and a minimum of 2 years of experience working with a social work agency
• Reside in Charlotte County

Qualifications

• BA/BS degree in a health care related field or equivalent background
• Minimum of 2 years of experience working with a social work agency

Nice to Have

• Bilingual in English/Spanish
• Long-term care experience
