Provide quality nursing care to patients in their homes under the direction of a registered nurse, assisting with assessment data collection and implementing care plans within the scope of licensure. This role involves working with an interdisciplinary team to offer personalized care and build rewarding relationships with diverse patients.
Responsibilities
- Provide quality nursing care to patients in their homes under the direction of a registered nurse
- Assist with collection of assessment data and provide input to the planning of care
- Implement plan of care within the scope of licensure
- Perform all duties in a manner which promotes team concept and reflects the Providence mission and philosophy
Requirements
- Graduate of an accredited LPN/LVN school of nursing
- National Provider BLS - American Heart Association within 30 days of hire
- Oregon Licensed Practical Nurse upon hire
- Washington Licensed Practical Nurse upon hire - if practicing within this state
- 1 year experience in nursing; long term care, acute care, ambulatory care or home health and hospice care
Qualifications
- Graduate of an accredited LPN/LVN school of nursing
- 1 year experience in nursing; long term care, acute care, ambulatory care or home health and hospice care
Nice to Have
- Experience in home health, hospice or public health
