LPN Licensed Practical Nurse Home Health

About This Role

Join a leading home health and hospice company as a Licensed Practical Nurse to provide direct patient care in their homes, collaborating with other healthcare professionals to ensure patient needs are met and documented accurately.

Responsibilities

• Verify plan of care with the RN prior to each visit
• Collaborate with physician and RN frequently to ensure patient needs are being met
• Notify physician and RN as soon as possible when patient condition changes
• Teach patients and caregivers self-care techniques
• Check vital signs
• Review all medications and perform a medication reconciliation at each visit
• Perform specialized procedures ordered by physician (e.g., foley catheter change)
• Prepare equipment and materials for treatments using the aseptic technique
• Complete documentation timely and accurately

Requirements

• One year of clinical LPN experience
• Current LPN license, specific to the state(s) you are assigned to work
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

Qualifications

• One year of clinical LPN experience
