Assist in developing and implementing wellness programs for patients in long-term care, memory care, or rehabilitation settings. This role involves completing assessments, care plans, and ensuring compliance with regulatory guidelines.

Responsibilities

• Develop and implement well-rounded wellness programs for Long Term Care, Memory Care, or Rehab patients
• Complete assessments, care plans, and associated paperwork in a timely manner
• Ensure all local, State, and Federal guidelines regarding programming are met

Requirements

• High School Diploma or GED
