Licensed Vocational Nurse, Home Health

About This Role

As a Home Health Licensed Practical Nurse (LPN), you will provide personalized, one-on-one nursing care to patients in their homes, helping them regain independence. You will work under the direction of a Registered Nurse, developing and managing care plans.

Responsibilities

• Provide professional nursing services under the direction of a Registered Nurse in compliance with state's Nurse Practice Act and healthcare standards
• Provide skilled nursing interventions for patient's illness, rehabilitative needs, and preventative care
• Utilize a holistic approach for problem-specific therapeutic interventions, teaching, and training activities according to the plan of care
• Apply knowledge and skills in accordance with accepted clinical practice standards to facilitate problem resolution and achieve individualized patient goals
• Confer with the RN Supervisor regarding needed changes in the Plan of Treatment and communicate verbal orders from physicians
• Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care
• Maintain contact with patients, physicians, and other healthcare team members regarding patient/family needs and status changes
• Participate in care coordination activities and discharge planning as appropriate
• Maintain the highest standards of professional conduct in relation to confidential information
• Prepare, submit and maintain documentation as required by the Company and/or facility

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid driver's license, auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
