Licensed Practical Nurse, Home Health

About This Role

As a Home Health LPN, you will provide personalized, one-on-one care to patients in their homes, helping them regain independence. You will work within a dedicated team of clinicians to develop and manage care plans, supporting patient recovery and well-being.

Responsibilities

• Provide professional nursing services under the direction of an RN, complying with state regulations and healthcare standards.
• Provide skilled nursing interventions for patient illness, rehabilitative, and preventative care, using a holistic approach.
• Apply clinical knowledge and skills to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor on changes to the Plan of Treatment and communicate physician orders where permitted.
• Implement the nursing plan of care using a systematic, individualized, goal-driven approach.
• Maintain timely contact with patients, physicians, and other healthcare team members regarding patient needs and status changes.
• Participate in care coordination and discharge planning as appropriate.
• Maintain professional conduct regarding confidential information, sharing only when access is established and in the patient's best interest.
• Ensure adherence to professional standards for the availability of supplies and equipment.
• Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered.

Requirements

• Graduate of an accredited LPN/LVN Program
• Current nursing license in practicing state
• Valid driver's license, auto insurance, reliable transportation
• Current CPR certification
• 2 years experience as LPN/LVN in a clinical setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• 2+ years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
