Licensed Practical Nurse Home Health

About This Role

Provide personalized, one-on-one home healthcare as an LPN, helping patients regain independence and managing care plans in collaboration with a dedicated team of physicians and clinicians.

Responsibilities

• Provide professional nursing services under the direction of an RN in compliance with state Nurse Practice Act and healthcare standards
• Deliver skilled nursing interventions for patient illness, rehabilitative needs, and preventative care
• Utilize a holistic approach in problem-specific therapeutic interventions, teaching, and training
• Apply clinical practice standards to facilitate problem resolution and achieve individualized patient goals
• Confer with RN Supervisor regarding plan of treatment changes and accept verbal physician orders when permitted
• Utilize a systematic, individualized goal/outcome-driven approach in implementing the nursing plan of care
• Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes
• Participate in care coordination activities and discharge planning as appropriate
• Maintain the highest standards of professional conduct regarding confidential information
• Prepare, submit, and maintain documentation as required, ensuring visit/shift notes are documented on the day services are rendered

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid driver's license
• Auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
