Licensed Practical Nurse, Home Health

About This Role

This role involves providing personalized and comprehensive home health care as a Licensed Practical Nurse, working under the direction of a Registered Nurse. You will help patients regain their independence and manage care plans to support recovery in their homes.

Responsibilities

• Provide professional nursing services under the direction of an RN, complying with state Nurse Practice Act and healthcare standards.
• Deliver skilled nursing interventions for patient illness, rehabilitative, and preventative care.
• Utilize a holistic approach for therapeutic interventions, teaching, and training activities in accordance with the care plan.
• Apply clinical practice knowledge and skills to achieve individualized patient goals and outcomes.
• Confer with the RN Supervisor regarding Plan of Treatment changes and communicate physician orders to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized goal/outcome driven approach.
• Maintain timely contact with patients, physicians, and other healthcare team members regarding patient needs and status changes.
• Participate in care coordination activities and discharge planning.
• Maintain professional conduct regarding confidential patient information.
• Prepare, submit, and maintain required documentation, ensuring visit/shift notes are completed on the day services are rendered.

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
