Provide personalized, one-on-one home health nursing care to help patients regain independence. Develop and manage care plans in collaboration with physicians and clinicians to support patient recovery.
Responsibilities
- Provide professional nursing services under the direction of a Registered Nurse in compliance with state Nurse Practice Act and healthcare standards.
- Deliver skilled nursing interventions for patient illness, rehabilitative needs, and preventative care.
- Apply knowledge and skills to facilitate problem resolution and achieve individualized patient goals.
- Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate verbal orders from physicians.
- Utilize a systematic, individualized goal/outcome-driven approach in implementing the nursing plan of care.
- Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
- Participate in care coordination activities and discharge planning.
- Maintain the highest standards of professional conduct regarding confidential patient information.
- Ensure appropriate communication regarding supplies, materials, and equipment needed for safe and effective care.
- Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered.
Requirements
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
- Current nursing license in the practicing state
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
Qualifications
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
- 2+ years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
