Licensed Practical Nurse Home Health

About This Role

Provide personalized, one-on-one home health nursing care to help patients regain independence. Develop and manage care plans in collaboration with physicians and clinicians to support patient recovery.

Responsibilities

• Provide professional nursing services under the direction of a Registered Nurse in compliance with state Nurse Practice Act and healthcare standards.
• Deliver skilled nursing interventions for patient illness, rehabilitative needs, and preventative care.
• Apply knowledge and skills to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate verbal orders from physicians.
• Utilize a systematic, individualized goal/outcome-driven approach in implementing the nursing plan of care.
• Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
• Participate in care coordination activities and discharge planning.
• Maintain the highest standards of professional conduct regarding confidential patient information.
• Ensure appropriate communication regarding supplies, materials, and equipment needed for safe and effective care.
• Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered.

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid driver's license
• Auto insurance
• Reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• 2+ years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
