Licensed Practical Nurse, Home Health

About This Role

Provide personalized, one-on-one home health care to patients, helping them regain independence and manage care plans under the direction of an RN. Utilize skilled nursing interventions and a holistic approach to support patient recovery.

Responsibilities

• Provide professional nursing services under the direction of a RN in compliance with state regulations and company policies.
• Provide skilled nursing interventions for patient illness, rehabilitative needs, and preventative care.
• Apply clinical practice standards to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate physician orders.
• Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
• Maintain timely contact with patients, physicians, and other healthcare team members regarding patient needs and status changes.
• Maintain the highest standards of professional conduct regarding confidential patient information.
• Ensure adherence to professional standards in the provision of supplies, materials, and equipment for patient care.
• Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered.

Requirements

• Graduate of an accredited LPN/LVN program
• Current nursing license in practicing state
• Valid driver's license, auto insurance, reliable transportation
• Current CPR certification
• Two years LPN/LVN experience in a clinical setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
