Licensed Practical Nurse, Home Health

About This Role

Provide personalized, one-on-one home health care to patients, helping them regain independence and health. Collaborate with a dedicated team of physicians and clinicians to develop and manage care plans.

Responsibilities

• Provide professional nursing services under the direction of a Registered Nurse in compliance with state Nurse Practice Act and healthcare standards
• Administer skilled nursing interventions for patient illness, rehabilitation, and preventative care, using a holistic approach
• Apply clinical practice knowledge to facilitate problem resolution and achieve individualized patient goals
• Confer with RN Supervisor regarding plan of treatment changes and communicate physician orders
• Utilize a systematic, individualized goal/outcome driven approach in implementing nursing care plans
• Maintain timely contact with patients, physicians, and healthcare team regarding patient needs and status changes
• Participate in care coordination activities and discharge planning
• Maintain confidentiality of patient information, sharing only when clearly in the patient's best interest
• Ensure adherence to professional standards for supplies, materials, and equipment needed for care
• Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered

Requirements

• Graduate of an accredited LPN/LVN program
• Current nursing license
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• 2+ years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
