Join Easterseals PORT Health's Assertive Community Treatment (ACT) Team as a Licensed Mental Health Therapist, providing impactful, community-based therapy to adults with severe mental health and substance use needs. You will work within a multidisciplinary team to deliver treatment, stability, and hope. This role makes a real difference in clients' lives, championing choice and dignity through trauma-informed, person-centered care.
Responsibilities
- Provide individual and group therapy to 4–5 adult clients per day in the community
- Create individualized treatment plans aligned with ACT best practices
- Participate in daily multidisciplinary team meetings
- Collaborate with families, community partners, and referring agencies
- Maintain timely, accurate documentation in Welligent EMR
- Participate in the rotating on-call crisis schedule as part of ACT model fidelity
- Uphold ethical standards, trauma-informed care principles, and client-centered practice
Requirements
- Licensure: LCSW/A, LCMHC/A, LMFT/A (provisional or full NC licensure required)
- Comfortable meeting clients where they are
- Valid driver’s license
- Reliable transportation
- Passion for serving vulnerable adults
- Strong Interpersonal Skills
