Lead Caregiver

About This Role

Improve the quality of care for clients living with dementia by training staff, covering short-notice shifts, and supporting Care Coordinators with care management tasks.

Responsibilities

• Start new client cases by working the first three shifts with all new clients in the assigned region
• Provide feedback on care plans to Care Coordinators
• Train and introduce Memory Care Professionals (MCPs) for ongoing client teams
• Cover MCP callouts and all short-notice call-outs during assigned schedule
• Train and coach MCPs on client cases
• Assist Care Coordinators with care plan meetings for MCPs as needed
• Introduce all MCPs to clients on their first shift
• Assess MCP readiness and work the shift for MCP if necessary
• Support Care Management with tasks such as buying groceries and picking up prescriptions for clients
• Work office shifts at the start of scheduled office shifts when not with a client, completing assigned tasks

Requirements

• Minimum of 2 years of professional caregiving experience
• Dementia care experience
• Ability to lift a minimum of 50 pounds
• Valid Driver’s License and vehicle with current registration and insurance
• Reliable means of transportation to get to and from work
• Ability to pass a background check
• Proof of eligibility to work in the U.S.
