About This Role

Deliver exceptional in-home skilled nursing care to aging seniors by executing personalized care plans and providing prescribed medical services, while supporting clients and families with their nursing needs.

Responsibilities

• Deliver exceptional care to dedicated clients in their home
• Help execute comprehensive personalized weekly schedules and care plans
• Provide skilled nursing services as prescribed under a client's plan of care, safely in the home, within your scope of practice
• Counsel clients and family in meeting nursing and related needs
• Meet all documentation requirements according to agency policy and procedure
• Coordinate services by participating in case conferences, case management, team meetings, and staff meetings
• Document completion of tasks and report changes in the client’s physical condition, mental capability, or behavior

Requirements

• Registered Nurse (RN) license
• BLS certification
• Experience in placing peripheral IV's, accessing ports and working with PICC lines
• Ability to use sound judgement and remain calm in a variety of situations
• Basic computer skills
• Proof of eligibility to work in the U.S.
