The Intensive Care Manager provides continuous treatment team care management services to consumers with severe and persistent mental illness. This role involves assessing consumer needs, developing individualized service and crisis plans, and linking clients to community resources while ensuring timely documentation and adherence to productivity standards.
Responsibilities
- Perform continual assessment of consumers’ needs and develop individualized service and crisis plans with measurable goals.
- Assist with referral and linkage to community resources and advocate for consumers.
- Participate in a rotating on-call schedule, including after-hours and weekends.
- Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.
- Plan and coordinate consumers' daily psychiatric, medical, and environmental needs.
- Ensure efficient transition of consumers within the Agency and community.
- Meet productivity standards and regularly meet with the Care Management Supervisor.
- Attend morning and weekly team meetings and follow-up on plans developed.
- Facilitate education of consumers and/or involved collaterals regarding psychiatric diagnosis and appropriate treatment options.
- Respond to urgent/crisis consumer needs with supervisor, crisis services, and clinic staff assistance.
- Develop and maintain relationships within family units and friends.
- Ensure timely and appropriate documentation as required by the Agency.
- Develop and maintain community relations through prompt follow-through.
- Complete individualized review (concurrent review) for payors based on consumer’s needs.
- Participate in rotation to facilitate daily group activities for consumers involved with CTT.
- Perform all duties associated with Care Manager I.
Requirements
- Bachelor’s degree (B.A., B.S.) in Mental Health related field
- Valid Tennessee Driver’s License
- Acceptable Motor Vehicle Report (MVR)
- Acceptable Criminal Background Investigation
- Personal Automobile Insurance
