Provide direct care and engaging activities for adult participants in their homes, following individualized care plans to ensure independence and well-being. Document all care, activities, and observations in the participant's electronic health record.
Responsibilities
- Attend to the daily needs of clients on an individual basis
- Understand and adhere to established policies and procedures
- Perform personal care, bathing, and hands-on care as assigned
- Complete housekeeping tasks as assigned, including sweeping, mopping, vacuuming, and cleaning bathrooms
- Provide opportunities for leisure activities and foster independence in choice and participation
- Complete appropriate visit records by the end of each shift
- Adapt and modify activities to better serve each client
- Ensure participant safety in the home
- Provide ongoing support, guidance, role modeling, and supervision to clients
- Provide a supportive environment for individuals to determine personal preferences for daily schedules and routines
- Maintain knowledge of ongoing issues and developments, and inform supervisor of concerns
- Communicate in a professional manner
- Execute duties to reflect reasonable safety standards, utilizing universal/standard precautions
- Remain alert at all times throughout scheduled work shift
- Participate in core training and maintain required certifications
- Provide crisis intervention and access emergency services as needed
- Attend and actively participate in staff meetings
- Ensure clients are treated with dignity and respect
- Assist the Director of Home Care where needed
Requirements
- Qualifying criminal background and driving record
- Ability to pass a drug screen
- Ability to provide or obtain a 2 step PPD
- Ability to lift 50 or more pounds
- Ability to demonstrate competence in certain areas of training (CPR, First Aid, non-violent physical interventions, behavior management, ADL care and management, driving)
- Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations
- Ability to react to situations such as rescue and preventing an individual from harming themselves
Qualifications
- Direct care experience working with geriatric and/or adults diagnosed with Intellectual Disabilities preferred but not required
Nice to Have
- Direct care experience working with geriatric and/or adults diagnosed with Intellectual Disabilities
- Valid driver's license in state of residence
