This role involves overseeing all aspects of medical care for Guests, performing health assessments, and implementing medical care plans to ensure optimal independence and well-being within an adult day center.
Responsibilities
- Perform head to toe physical assessments of current and prospective Guests upon admission and annually
- Develop and implement medical plan of care
- Complete and document regular monitoring of vital signs, weight, blood sugar, medication administration, bowel and bladder care, and education
- Develop and implement programs in bowel and bladder care, restorative feeding, ambulation, range of motion, personal living skills, transfers, and showering with Guests
- Update and maintain Guest medical files, including medical plan of care, care records, and progress notes
- Assist Guests and supervise Guest staff in assistance with Activities of Daily Living (ADLs)
- Observe and report Guest conditions, reactions, and responses to the Regional/Assistant Regional Director of Medical Services and/or Center Executive Director
- Maintain confidentiality, Guest privacy, and dignity in all care
- Ensure security of, and proper documentation for all Guest medication stored and administered at the Center
- Collaborate with other IADCC medical staff on Guest Health Care Coordination, including medical record consolidation and medication management
Requirements
- LPN or RN license
- Valid driver's license in state of residence
- Qualifying criminal background and driving record
- Certification/experience in CPR
- Certification/experience in First Aid
- Certification/experience in non-violent physical interventions
- Certification/experience in behavior management
- Certification/experience in medication administration
- Certification/experience in driving
Qualifications
- Direct care experience working with geriatric and/or adults diagnosed with Intellectual Disabilities preferred.
