Hospice Registered Nurse (RN)

About This Role

The Hospice Registered Nurse (RN) oversees nursing care, coordinates with other healthcare professionals, and develops care plans for patients in a home healthcare setting. This role is responsible for educating patients and families and fulfilling night and weekend duties.

Responsibilities

• Oversee nursing care team
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care
• Plan, organize, and direct home care services
• Conduct assessments of patient care needs and develop care plans
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes
• Prepare timely, complete, and accurate clinical notes
• Work night and weekend shifts as required per hospice needs of patients

Requirements

• Valid RN license in the State of Texas
• Hospice experience (at least 1 year)
• Knowledge of home health regulations and general healthcare compliance
• Excellent communication and people skills
• Ability to work independently and as part of a team

Qualifications

• At least 1 year of hospice experience

Nice to Have

• Experience with Home Care Home Base (HCHB) EMR
