Clinical Branch Manager (RN) Home Health

About This Role

This role involves planning, coordinating, and delivering quality interdisciplinary home health care services for Transition Home Healthcare, ensuring compliance with federal/state standards and clinical best practices. The Branch Manager will supervise a team of healthcare professionals and oversee patient case management and outcomes.

Responsibilities

• Supervise, monitor, and ensure competencies and performance of assigned home health care team(s)
• Supervise ongoing case management of all patients, ensuring accurate evaluation and treatment plans, maintaining quality models, and utilizing services appropriately
• Review electronically submitted documentation and provide feedback to clinicians regarding deficiencies or OASIS corrections
• Review agency assigned team outcome data, and participate in and facilitate improvement activities for agency quality and outcome performance
• Review patient schedules to ensure appropriate coordination of care and clinical needs and efficiency of clinical associates
• Serve as an educator and mentor for the home health care team regarding documentation, clinical standards, and compliance

Requirements

• 2+ years experience as Home Health Registered Nurse (RN) Supervisor
• Supervisory Experience in Home Health
• Ability to reliably commute to and work out of the Branch
• RN in Texas
• Knowledge of Home Health regulations and general healthcare compliance
• Proficient in all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
• Managed a team of RN, LVN, HHA, PT, OT, PTA, SLP, and COTAs

Qualifications

• 2+ years experience as a Home Health Registered Nurse (RN) Supervisor
