About This Role

This role is for a Hospice Registered Nurse (RN) who will provide personalized, meaningful hospice and palliative care to patients and their families, guiding them through their end-of-life journey with dignity and comfort.

Responsibilities

  • Serve as the primary point of contact for patients and families, guiding them through the hospice journey
  • Deliver and document skilled, hands-on nursing care based on individualized Plan of Care in various settings
  • Perform initial and ongoing assessments to monitor patient condition and adjust care as needed
  • Collaborate with an interdisciplinary team (physicians, LPNs, CNAs, social workers, chaplains) to refine care plans
  • Provide emotional, physical, and spiritual support to patients and their loved ones, including bereavement guidance
  • Educate caregivers on safe and confident care for their loved ones, offering practical training and emotional reassurance
  • Communicate important updates in patient condition to the care team for timely interventions
  • Maintain accurate clinical documentation and follow infection control and compliance standards
  • Support quality improvement and scheduling initiatives
  • Educate patients and families on medication administration, hospice philosophy, symptom/pain management, and end-of-life processes

Requirements

  • Active RN license in the state of employment (or eligible to obtain)
  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
  • Compassionate, empathetic, and patient-centered approach to care
  • Comfortable providing care in diverse settings (private homes, facilities)
  • Strong communication and critical thinking skills in emotionally sensitive situations
  • Valid driver’s license
  • Auto insurance
  • Reliable transportation
  • CPR certification

Qualifications

  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields

Nice to Have

  • Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
  • Admissions RN experience
  • Wound care experience
  • Float pool experience
  • Travel nursing experience
  • Experience with terminally ill patients or serious illness support
