Home Health Registered Nurse

About This Role

The Home Health Registered Nurse provides personalized, one-on-one skilled nursing care to patients in their homes, helping them regain independence. This role involves developing and implementing individualized care plans, monitoring patient conditions, and educating patients and families on disease management.

Responsibilities

• Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes to the patient's physician and Clinical Manager.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.

Requirements

• Diploma, Associate or Bachelor's Degree in Nursing
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Med surg, ICU, ER, or acute experience
• Experience collaborating with a team of healthcare professionals
• Valid driver's license, auto insurance and reliable transportation

Qualifications

• Diploma, Associate or Bachelor's Degree in Nursing
• One year nursing experience, with preferred home health experience. Experience in med surg, ICU, ER, or acute settings is required.
