This role is for a Registered Nurse Clinical Manager in Home Health responsible for managing and coordinating patient home care services from admission to discharge. You will plan and coordinate the delivery of patient care plans according to physician orders.
Responsibilities
- Manage and coordinate appropriate home care services from admission to discharge
- Plan and coordinate the delivery of each assigned patient's plan of care in accordance with physician orders
Requirements
- Current, valid, and unencumbered Tennessee RN license or Compact State licensure
- Current BLS/CPR
- Current and valid driver license, transportation, and auto insurance
- Two or more years home health experience required
- One or more years EMR experience required
- Computer proficiency
- Self-motivated
- Customer service oriented
Qualifications
- 2+ years home health experience, 1+ year EMR experience
