This role involves providing personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You will implement individualized care plans, educate patients and families, and maintain accurate records while collaborating with the healthcare team.
Responsibilities
- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN
- Implement individualized care plans in collaboration with the patient, family, and healthcare team
- Educate patients and their families on disease management, treatment options, and self-care techniques
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
Requirements
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- TB screening
Qualifications
- Two years experience as an LPN/LVN in a clinical setting
Nice to Have
- Nursing experience in a Home Health or Hospice setting
