Provide skilled nursing care to home health patients, implementing individualized care plans, educating patients and families, and maintaining accurate records to help them regain independence.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN
- Implement individualized care plans in collaboration with the patient, family, and healthcare team
- Educate patients and their families on disease management, treatment options, and self-care techniques
- Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes
- Maintain contact with patients, physicians, clinical managers, and other members of the healthcare team regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
Requirements
- Two years experience as an LPN/LVN in a clinical setting
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
Qualifications
- Two years experience as an LPN/LVN in a clinical setting
Nice to Have
- Nursing experience in a Home Health or Hospice setting
