Home Health Licensed Practical Nurse

About This Role

Provide skilled nursing care to home health patients, implementing individualized care plans, educating patients and families, and maintaining accurate records to help them regain independence.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN
• Implement individualized care plans in collaboration with the patient, family, and healthcare team
• Educate patients and their families on disease management, treatment options, and self-care techniques
• Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes
• Maintain contact with patients, physicians, clinical managers, and other members of the healthcare team regarding patient or family needs and status changes
• Participate in care coordination activities and discharge planning

Requirements

• Two years experience as an LPN/LVN in a clinical setting
• Current nursing license in the practicing state
• Valid drivers license
• Auto insurance
• Reliable transportation
• Current CPR certification

Qualifications

• Two years experience as an LPN/LVN in a clinical setting

Nice to Have

• Nursing experience in a Home Health or Hospice setting
