Home Health Licensed Practical Nurse

About This Role

This role primarily focuses on providing skilled nursing care to patients in their homes under the supervision of a Registered Nurse. The purpose of this role is to implement individualized care plans, educate patients and families on disease management, and maintain accurate records to help patients recover and regain independence.

Responsibilities

• Provide skilled nursing care to patients by working on-site in their homes, under the direction of an RN
• Implement individualized care plans in collaboration with the patient, family, and healthcare team
• Educate patients and their families on disease management, treatment options, and self-care techniques
• Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
• Participate in care coordination activities and discharge planning

Requirements

• Current nursing license in the practicing state
• Valid drivers license
• Auto insurance
• Reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Two years experience as an LPN/LVN in a clinical setting

Nice to Have

• Nursing experience in a Home Health or Hospice setting
