Home Health Licensed Practical Nurse

About This Role

Provide personalized, one-on-one skilled nursing care for patients in their homes, helping them regain independence and manage chronic conditions or recover from injury. Collaborate with a dedicated healthcare team and educate patients and families on disease management and self-care techniques.

Responsibilities

• Provide skilled nursing care to patients in their homes, under the direction of an RN
• Implement individualized care plans in collaboration with the patient, family, and healthcare team
• Educate patients and their families on disease management, treatment options, and self-care techniques
• Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes
• Maintain prompt contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient or family needs and status changes
• Participate in care coordination activities and discharge planning

Requirements

• Current nursing license in the practicing state
• Valid driver's license
• Auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Two years experience as an LPN/LVN in a clinical setting

Nice to Have

• Nursing experience in a Home Health or Hospice setting
