Home Health Licensed Practical Nurse

About This Role

The Home Health Licensed Practical Nurse provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. This role involves implementing individualized care plans, educating patients and families on disease management, and maintaining accurate patient care records.

Responsibilities

• Provide skilled nursing care to patients in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.

Requirements

• Current nursing license in the practicing state
• Valid driver's license
• Auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Two years experience as an LPN/LVN in a clinical setting

Nice to Have

• Nursing experience in a Home Health or Hospice setting
