As a CenterWell Home Health LPN, you will provide personalized, one-on-one skilled nursing care to patients in their homes, helping them regain independence. You will manage care plans, educate patients and families, and collaborate with a dedicated healthcare team.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN
- Implement individualized care plans in collaboration with the patient, family, and healthcare team
- Educate patients and their families on disease management, treatment options, and self-care techniques
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
Requirements
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
Qualifications
- Two years experience as an LPN/LVN in a clinical setting
Nice to Have
- Nursing experience in a Home Health or Hospice setting
