Home Care RN

About This Role

Provide primary nursing care to home-based patients, assessing conditions, developing care plans, and ensuring adherence to medical orders and organizational policies. Collaborate with physicians and case managers to achieve positive patient outcomes and satisfaction.

Responsibilities

• Provide primary nursing care to home-based patients as ordered by the physician and in accordance with policies and procedures
• Responsible for initial assessment and care plan development in collaboration with the Population Health Case Manager
• Provide functional support and supervision to Home Healthcare Aides and LPNs as needed
• Provide therapeutic intervention, overall care management, and achieve client outcomes and patient satisfaction
• Accurately assess patient condition and risk at admission and required time points
• Develop assessment-based plan of care in collaboration with the physician, Population Health Case Manager, and patient
• Maintain current working knowledge of applicable Federal, State, and local laws and regulations, and agency policies and procedures
• Report changes in patient condition in a timely manner to the physician and/or Population Health Case Manager
• Obtain orders for changes in the plan of care to respond to the patient's condition
• Follow orders for treatment and provide services as indicated on the plan of care
• Utilize and implement appropriate educational tools and pathways per guidelines and/or policy and procedure

Requirements

• Graduate of an approved nursing education program
• Licensure as a Registered Nurse in the State of Connecticut
• One (1) year experience as a professional acute care nurse
• Current Driver’s license and reliable transportation

Qualifications

• Graduate of an approved nursing education program, BSN preferred
• One (1) year experience as a professional acute care nurse
