Home Care Occupational Therapist

About This Role

Provide professional occupational therapy services to clients in their homes within the Waterbury region, focusing on assessment, treatment, and coordination of care with a multidisciplinary team.

Responsibilities

• Evaluate and treat patients needing occupational therapy services in their homes
• Attend regular team meetings and coordinate care with other disciplines
• Assess clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation
• Develop appropriate assessment-based plan of treatment with physician and client, including attainable goals
• Make appropriate referrals for evaluation/care to other disciplines and services, and coordinate care
• Utilize interview, observation and evaluation in assessing clients and apply professional judgment to formulate interventions
• Evaluate the home environment and make recommendations for effective household management
• Initiate and implement appropriate preventive, therapeutic and rehabilitative procedures and techniques
• Assist and instruct clients and family members in activities and concepts to promote health, safety, and independent function

Requirements

• Graduate of an accredited occupational therapy curriculum
• Eligible for NBCOT National Registration Examination
• NBCOT registration
• CT Occupational Therapist registration
• Minimum of one year experience
• Current Driver’s license and reliable transportation

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.
• Minimum of one (1) year experience, home healthcare experience preferred.
