Heart Wellness Registered Nurse

About This Role

This role involves following patients in the Heart Wellness Program via tele-management, screening, and tracking hospital patients for heart failure core measures. The HWRN functions as a clinician and educator for adult patients, overseeing program outcomes.

Responsibilities

• Manage care of complex cardiac patients within the scope of the Heart Wellness Program
• Utilize tele-management to identify problems and prevent hospital admissions, intervening according to Heart Wellness Program protocols
• Manage medication for inpatient and outpatient populations, making adjustments or recommendations with Primary Care Provider or Cardiologist
• Serve as a clinical resource for the organization in area of expertise
• Identify potential patients for Heart Wellness Program management by reviewing new admissions
• Provide educational sessions using adult learning principles to support patients, families, staff, and external organizations
• Collaborate with other disciplines to achieve patient care and organizational goals
• Produce monthly newsletter and conduct monthly educational and support programs
• Take ownership and responsibility for individual performance goals
• Involve self in departmental, team, and committee goals
• Complete departmental and hospital requirements

Requirements

• Graduate of an accredited Nursing program
• Licensed as a Registered Nurse in the State of NH
• Certified in Basic Life Support within 3 months of hire date
• A minimum of 3 years acute care clinical experience in cardiovascular nursing

Qualifications

• Graduate of an accredited Nursing program. Bachelor's in Nursing (BSN) preferred.
• A minimum of 3 years acute care clinical experience in cardiovascular nursing.
