About This Role

Provide advanced pharmaceutical care to FHC patients as a key member of an interdisciplinary team, focusing on direct patient care, medication therapy management, and chronic disease management.

Responsibilities

  • Conduct comprehensive medication reviews for patients with chronic illnesses
  • Collaborate with primary care providers to optimize pharmacotherapy and develop care plans
  • Provide direct patient-centered care and integrated pharmacy operational services
  • Provide medication counseling and adherence support directly to patients
  • Manage medication therapy under collaborative practice agreements
  • Participate in interdisciplinary team huddles and case conferences
  • Monitor patients for adverse drug reactions, interactions, and efficacy of therapies
  • Support the management and compliance of the 340B Drug Pricing Program
  • Serve as a preceptor to pharmacy students and residents, as applicable
  • Assist in the development and implementation of clinical protocols and formularies
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Serve as a key participant on FHC’s Quality Improvement (QI) Committee
  • Ensure pharmacy services comply with relevant regulatory bodies

Requirements

  • Doctorate degree in Pharmacy
  • Active Kentucky Pharmacy license in good standing
  • Immunization administration certification
  • Naloxone certification
  • Ability to provide own transportation to work at other locations
  • Willingness to work extended hours as necessary
  • Willingness to work every other Saturday from 9:00 a.m. to 1:00 p.m.

Qualifications

  • Doctorate degree in Pharmacy

Nice to Have

  • 340B University completed within the last two years
  • Experience at a 340B outpatient pharmacy
  • ASHP or ACCP PGY1 accredited residency
  • Ability to fluently communicate both verbally and in writing with Spanish-speaking patients about their medication
