About This Role

This role involves providing direct clinical support to older adults through wellness assessments and initial therapy sessions at partner residential locations, connecting them to ongoing care. The coordinator will represent Total Life in the community as a compassionate clinical contact for clients, caregivers, and partners.

Responsibilities

  • Complete initial clinical sessions and develop recommendations for ongoing therapy or support
  • Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being
  • Represent Total Life in the community and at residential partner sites as the local point of contact for clients, caregivers, and partners
  • Collaborate with our virtual therapy team to ensure smooth transitions into ongoing care
  • Maintain accurate and timely clinical documentation
  • Participate in team meetings and contribute to improving our community-based programs

Requirements

  • Active LCSW, LMFT, LPC, or LMHC license in Texas
  • Fully licensed with ability to perform without supervision
  • Strong background in evidence-based modalities
  • Comfortable meeting clients in community or residential settings
  • Professional liability insurance ($1M/$3M coverage) or willingness to obtain
  • Active NPI number
  • Ability to pass a criminal background check
  • Reliable transportation for local travel

Qualifications

  • Both recent graduates and experienced professionals are welcome.

Nice to Have

  • Experience working with older adults or in geriatric mental health
  • Strong interpersonal and communication skills with a compassionate, patient-centered approach
  • Bilingual in English/Spanish
  • Established relationships within your local community
