Responsibilities

• Assess patient needs throughout the episode of care
• Coordinate and plan patient care
• Direct and implement patient care and management
• Manage patient needs with a focus on progress toward discharge
• Oversee transitions within the acute care stay
• Responsible for the delivery and coordination of patient care provided by the caregiving team
• Assure competent, compassionate patient care is uniformly provided to customers, hospital staff, and patients
