About This Role

As a Clinical Navigator, you will serve as the hospital's representative, meeting prospective patients, assessing their needs, and communicating the potential benefits of rehabilitation. You will cultivate referral relationships, manage an assigned territory, and complete patient assessments while collaborating with internal and external teams.

Responsibilities

  • Cultivate referral relationships
  • Manage assigned territory
  • Complete patient assessments
  • Make sales contacts and analyze referral and admission data
  • Collaborate with business development team members, nursing, and other staff
  • Facilitate referral conversion
  • Provide feedback and recommendations for program development and quality improvement
  • Develop and maintain excellent relationships with stakeholders

Requirements

  • Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy
  • Minimum of two years’ experience in the designated field of license

Qualifications

  • Degree in a health-related field from an accredited college or university preferred
  • Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience

Nice to Have

  • Degree in a health-related field
