Clinic Director I Licensed Physical Therapist (PT) - Care Coordination

About This Role

Serve as a Clinic Director and Licensed Physical Therapist (PT) within a senior living community, overseeing care delivery and providing physical therapy and wellness services for older adults. This role combines clinical practice with care coordination, allowing for flexible scheduling and significant impact on seniors' well-being.

Responsibilities

• Oversee how team members at location(s) deliver care in accordance with federal, state, and local regulations
• Support care coordination efforts by conducting regular wellness checkpoint visits with patients
• Communicate with patients' providers and caregivers
• Assist patients with attending telehealth visits
• Lead operational initiatives, including the implementation of company-wide policies, clinical programs, and quality initiatives
• Advise interdisciplinary care partners on treatment planning and therapeutic interventions
• Maintain professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
• Provide physical therapy and wellness services for older adults
• Conduct assessments, treatment plannings, and therapeutic interventions as part of a multidisciplinary team
• Contribute to program development, quality improvement, and problem-solving
• Optimize each patient's functional well-being and satisfaction

Requirements

• Completion of an accredited Physical Therapy program
• Current state licensure in PT
• Excellent verbal and written communication skills
• A results-oriented mindset with critical thinking
• Tech-savvy skills
• Professional curiosity and a knack for finding solutions
