Clinic Director I Licensed Physical Therapist (PT) - Care Coordination

About This Role

This role is for a licensed Physical Therapist to serve as a Clinic Director, overseeing care delivery and leading operational initiatives in a senior living community. It combines traditional physical therapy with care coordination duties, offering flexible scheduling and significant impact on seniors' lives.

Responsibilities

• Oversee how team members at your location(s) deliver care in accordance with federal, state, and local regulations
• Support care coordination efforts by conducting regular wellness checkpoint visits with patients
• Communicate with patients’ providers and caregivers and assist patients with attending telehealth visits
• Lead operational initiatives, including the implementation of company-wide policies, clinical programs, and quality initiatives
• Advise interdisciplinary care partners on treatment planning and therapeutic interventions
• Maintain professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
• Provide physical therapy and wellness services for older adults
• Conduct assessments, treatment plannings, and therapeutic interventions as part of a multidisciplinary team
• Contribute to program development, quality improvement, and problem-solving
• Optimize each patient’s functional well-being and satisfaction

Requirements

• Licensed Physical Therapist (PT)
• Excellent verbal and written communication skills
• Results-oriented mindset with critical thinking ability
• Tech-savvy skills

Qualifications

• Completion of an accredited Physical Therapy program
