Case Worker

About This Role

The Case Worker provides care on the inpatient unit by conducting initial interviews, ongoing assessments, and incorporating family input. This role is responsible for initiating discharge and follow-up planning at the time of admission.

Responsibilities

• Provide care utilizing Case Worker Initial Interview
• Conduct ongoing assessment
• Incorporate family input into care planning
• Begin discharge planning at time of admission
• Initiate follow-up planning at time of admission

Requirements

• Bachelors Degree in Behavioral Health related or Human Services field (Psychology, Counseling, Rehab counseling, Social Work)
• Completion of a crisis prevention program or the equivalent

Qualifications

• Bachelor’s Degree in Behavioral Health related or Human Services field (Psychology, Counseling, Rehab counseling, Social Work)
• A minimum of 2 years experience in behavioral health setting preferred.

Nice to Have

• Master’s degree preferred
• A minimum of 2 years experience in behavioral health setting preferred
