Case Worker

About This Role

This Case Worker position provides clinical case management to children and families in a multicultural congregate setting, focusing on achieving permanent, long-term life plans for each child. The role involves intensive case management, direct services, and collaboration with community partners to advocate for children's needs.

Responsibilities

• Provide clinical case management to children and families
• Focus on achieving a permanent, long-term life plan for each child
• Conduct outreach and intensive case management
• Deliver direct services
• Collaborate with various systems and advocate for the child’s needs
• Develop and complete assessments, service plans, treatment plans, progress notes, and court reports in a timely manner

Requirements

• Bachelor's Degree in Social Work or a related field
• 1-3 years of experience working with children and families
• Valid driver’s license and the ability to transport minors if needed
• Successfully complete all required background checks
• Strong computer skills, including Microsoft Word and email

• Bilingual in Spanish and English
• Experience working with children who have social/emotional/physical challenges
• Knowledge of federal, state, program, and agency regulations
