Coordinate continuity of care for patients in an Acute Care setting, acting as a liaison between the patient's family and the healthcare organization to ensure proper treatment and cost-effective use of healthcare resources.
Responsibilities
- Coordinate continuity of care for patients often as a liaison between the patient’s family and healthcare organization
- Promote self-managed care and the use of healthcare resources in the most cost-effective way possible
- Ensure that the proper treatment is administered at the appropriate time in order to maximize health and well-being
Requirements
- Eighteen months of recent experience in an Acute Care Case Manager setting
- Active RN License
- BLS Certifications
- Degree from accredited nursing program
