This role involves responding to incoming referrals, conducting pre-screening assessments, and managing the intake process for clients seeking foster care services. The Case Manager will ensure quality care by coordinating with clinical teams and addressing client needs efficiently.
Responsibilities
- Receive and respond to incoming calls from referral sources and potential clients to identify needs and determine the Company's ability to meet them
- Complete viable referrals by setting up and carrying out pre-screening assessments, completing pre-screening reports, and preparing case proposals and other pre-admission paperwork
- Oversee, direct, and supervise field staff assisting with the pre-screening process
- Access national, state, and company account information, including account names and contract terms
- Consult with third-party representatives regarding client benefit coverage, financial responsibility, service authorization, and reimbursement procedures
- Contact referral sources to advise them of case acceptance and provide information on the clinical team responsible for the client's case
- Develop and maintain a working knowledge of all company and community services/resources, assisting in identifying alternative sources if needed
- Maintain relationships with standard referral sources and payer case managers, contacting identified sources for referrals and reporting outcomes
- Monitor and track referral source satisfaction levels, conversion ratios, and provide summary reports to management
- Implement and maintain operational processes to ensure compliance with Company policies, requirements, and regulatory mandates
- Adhere to and participate in the Company's mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs
- Participate in quarterly growth planning meetings and activities, including discussions around staffing and recruitment needs
Requirements
- Bachelor's degree in a human services field or nursing field
- Valid driver's license
- Two years of experience with case management or related discipline
- Ability to communicate both verbally and in writing
